Where to Start:
- Please call your pediatrician and request that a Medical Referral for the desired services you are seeking to be faxed to MP Health at (805) 823-4500. Please Note: Mental Health services are considered ‘self-referral’ services and do not require a medical referral. Please contact our office directly to start the process of Mental Health services.
- Once our office has received a medical referral for your child, we will contact you promptly to discuss future steps, and the process of getting your child started with services. Our office may call from various numbers so please check your voicemail regularly. Our office wants to make sure you don’t lose your spot!
Once we have all your information, our provider will be contacting you to set up an evaluation with therapy services.
